Premiums will be changed on the 11/15/2010 payday. Co-payments, annual maximums, and lifetime maximums remain the same as the current plan. NO BENEFITS HAVE BEEN REDUCED FOR SERVICES OUTSIDE THE 50 STATES (including the CNMI, Guam and overseas). Within the 50 states, no benefits have been changed provided you see a PPO participating provider. Listings of PPO providers are available at www.aetna.com/docfind.
